Items needed for new service requests at locations with collection activity.

AES Ohio offers payment assistance to customers who need help paying bills. However, sometimes our customers have unpaid bills that are turned over to a collection agency.

If you are moving to a location with collection activity, you will need to provide two documents to show that you did not occupy the property during the time the past due balance was accumulated, or that the previous customer, who has outstanding bills, no longer occupies the property.

We want to make the process as easy as possible for you, so we have listed your options below for the required documents.

Documents to provide AES Ohio

 

Residential

Please provide the following documents, based on your relationship with the property. 
 
Tenant Property owner
1. Signed and notarized affidavit (both the notary's stamp and seal must be clearly readable)

and

2. One of the following:

  • Rental or lease agreement, including the date the lease was initiated, list of all occupants and signature page
  • Notarized landlord statement (both the notary's stamp and seal must be clearly readable)

and

2. Proof of ownership of the home (one of the following):

  • Settlement statement
  • Tax bill
  • General warranty deed (many county websites provide proof of ownership information)

 

Non-Residential

Please provide the following documents, based on your relationship with the property.
